A spokesperson for the actress confirmed to the New York Times that Holmes had been approached to star in the production.
The New York daily also reports that producers Debbie Bisno and Ray Gaspard are hoping to mount the psychological drama on Broadway later this season or during the 2011-12 season. Fences' Kenny Leon will direct.
Chloe Sevigny, the Times says, is also under consideration for the lead role of a woman "who turns the table on her would-be rapist and then struggles along with her two roommates about what to do with him."
Extremities made its Off-Broadway debut in 1982 at the Westside Theatre with Susan Sarandon in the lead role. Farrah Fawcett later succeeded Sarandon and subsequently played the role on film. The upcoming production would mark the Broadway debut of Extremities.
Katie Holmes has been seen on screen in "Batman Begins," "Disturbing Behavior," "Teaching Mrs. Tingle," "Pieces of April" and "Dawson's Creek."
